 CHAPERONE RESPONSIBILITIES FORM (PDF)
This form if for all trip chaperones. This form neeeds to be signed by
each Chaperone who particpates in every trip (local as well as intra- and inter-state) and turned into the Travel
Coordinator.
VANSD ACTIVITY CLEARANCE PACKET (PDF)
An Authorization for Emergency Medical Treatment, Permission to Participate in
Intersholastic Athletics (which includes Choir), Authorization for the Administration of Medication
in school, and Preparticipation History and Physical Examination is required for each student. These
forms are required by the Vancouver School District prior to each trip that may be taken (outside of
the local area) by each choir member. This packet can be downloaded from our site.
 SVMPA STUDENT PHOTO RELEASE FORM (PDF)
A Photo Release Form is required to be signed by a parent or guardian for any student (under the age of 18)
whose picture our group would like to use on our website or other promotional activities outside of
performances. The form needs to be filled out completely and returned to the Booster Organization
directly (e.g., at a meeting) or indirectly by placing it in the "Mailbox" in the Choir Room.

VANSD VOLUNTEER PACKET (PDF)
Both a Vancouver School District (VANSD) Volunteer Driver Statement and a Washington State Patrol Background
Check Form are required for any person who will be transporting students between events (with the exception of
their own children). These forms have been combined in a Volunteer Packet and need to be filled out completely and returned to the Skyview High School Front
Office. These forms can be downloaded from our site.
AUCTION DONATION REQUEST LETTER (PDF)
If you are helping out with donations for our upcoming 5th Annual Dinner Auction this November this form letter will help open doors and let the company know who we are and why they should donate. |
